Although you cannot apply any insurance benefits directly to your order on this site, your insurance company will reimburse you for the purchase, provided you have an applicable out-of-network benefit in your plan.   2. Subscriber’s name, address, and Spectera ID#
  3. Patient’s name and date of birth
  4. A note requesting reimbursement

VSP will reimburse you for your purchase of contact lenses at LensCraftersContacts.com if you have an out-of-network benefit included in your plan. To receive your reimbursement, simply submit your itemized receipt from your order along with a VSP out-of-network reimbursement form to VSP. The reimbursement form can be found on VSP’s website.
